Registration form
Click here for the registration form for 2010-2011. You will need to complete this and bring it to the church office. PLEASE DO NOT MAIL OR FAX, UNLESS YOU ARE TOLD OTHERWISE.
Registration period
During the last month of each year, registration forms are sent home with the students to each family. The normal registration period begins on May 15th and ends on August 15th.
You do not need an appointment to register, but you need to come in person to our offices. Our normal office hours 8:30-4:00pm Monday through Friday. Please note that the office closes each day from 12-1pm for lunch.
Late Registration Fee
All registrations for returning families taken after Aug.15th will be assessed a $40 late fee. This is added onto the regular tuition cost and it applies to each family, not each student.
Tuition
The tuition fee for religious education classes at St. Mary Nativity is $150.00 per child, and $20 for each additional child. St. Mary Nativity parishioners pay $100 per child and $20 for each additional child. Checks are to be made payable to St. Mary Nativity Church.
The tuition covers the costs of textbooks, instructional materials and supplies for each student. This also includes music, audio-visual materials, and operational office expenses. Tuition is subject to change from year to year. Generally, it will not be changed during the year. In cases of financial hardship, please discuss the situation with the Director of Religious Education.
Guidelines
- You must register in person at the church rectory or school office. We do NOT accept registrations by mail. Registrations by mail are considered null and void.
- Fill out the registration form COMPLETELY (front and back).
- If you are a parishioner of St. Mary Nativity or Holy Cross Parish, make sure you know your envelope number so that you can write it on the registration form. If you do not have an envelope number, then you are not a registered member of the parish.
- Make sure we have copies of baptismal certificates and First Communion certificates. If they received their sacraments here, you will have to verify this with the office secretary. If you want, you can call 815-726-4031 before registering.
- Registrations forms that are not fully completed will be returned and considered void until we have all the required information on paper. So please make sure you have everything you need, so you don’t have to make a second trip to the parish office.
- Bring your checkbook to pay tuition. Tuition costs have not changed, but please note that sacramental fees will be covered during the mandatory parent meetings for 1st Communion and Confirmation. Check your calendar for dates.
- After you turn in all required documents, you will receive:
- The 2010-2011 Class Schedule and Calendar
- The school handbook that includes all program policies and procedures. Please read it thoroughly to see if there are any changes.
- Upon receipt of the handbook, you will have to sign the acceptance form, stating that you have received the school handbook and that you follow the directives mentioned therein.
School Transfers
We are constantly receiving new students into our program from other schools and parishes. We ask that you bring copies of ALL your children's sacramental records (certificates) and get an official transcript from their previous school. For your information, a transcript normally states your child's attendance and/or grades, and also confirms the completion of the grade and school year for which they were enrolled. You must bring these papers to registration.
